Even the Best MTG Players Make Dumb Mistakes

From the Japanese Nationals, widely considered to be one of the toughest Magic The Gathering national tournaments:
Wanting to keep the pressure on Takahashi, Masashi was forced to use a Pact of Negation on a Cryptic Command from his opponent targeting an un-suspending Riftwing Cloudskate. Conspicuously he failed to put a marker on the top of his deck to remind him to pay the requisite 3UU during his following upkeep.
Yuuta’s turn went long after a series of plays and, sure enough, by the time Masashi got the turn back he had completely forgotten about his Pact. With an audible gasp Oiso drew for the turn as the audience could only watch in horror. Immediately realizing what he had done Oiso shuffled up his cards and conceded to Takahashi.
Masashi Oiso is one of the best players to ever play Magic The Gathering. He is eligible to be voted in the Pro Tour Hall of Fame in the class of 2012.
Amazingly, even with his bonehead play, he still won the whole tournament!

Gab Nassif has made a number of conspicuously boneheaded plays in high-level competition — my favorite being when he Mindslavered an opponent and screwed up his mana math at PT Kobe. From the coverage:
“Gabriel attacked for four and activated Mindslaver. He got to see a hand of three artifact lands and a Skullclamp. Gabriel/Hans played the Skullclamp and considered equipping the Hoverguard to run it into his Blinkmoth. He could not bring himself to give Hans two cards and merely attacked the Hoverguard into his land. Gabriel no extra mana available after activating the Blinkmoth and could not pump the Blinkmoth to a 2/2. He had not realized this immediately and was embarrassed by his misplay.
He had no answer for the flier on his next turn and lost the game much to his chagrin.”
He nonetheless won that match, and went on to second-place the whole tournament. (In contrast, his misplay in the quarters at Worlds 2007 may have cost him that match, and a shot at a Worlds title.)
Lol, talk about a Mindslaver that totally helps your opponent.
